Post-pubertal Gynecologic Evaluation and Management of Patients with Anorectal Malformations.
Journal of pediatric surgery
2026: 163293
Abstract
The Pediatric Colorectal and Pelvic Learning Consortium (PCPLC) is a group of medical providers dedicated to equitably improving the health and quality of life of individuals affected by congenital colorectal disorders and associated urologic, gynecologic, and other anomalies. Using research and medical expertise, the Gynecology Committee within the PCPLC has developed five consensus statements on gynecologic care of patients born with anorectal malformations: Gynecologic Care in Patients with Anorectal Malformations: a Primer and Call to Action, Methods of Gynecologic Evaluation for Patients with Anorectal Malformations, Prepubertal Gynecologic Evaluation and Management of Patients with Anorectal Malformations, Post-pubertal Gynecology Evaluation and Management of Patients with Anorectal Malformations, Long-term Obstetric and Gynecologic Care for Patients with Anorectal Malformations. In this article, we review pubertal trajectory and necessary evaluation of gynecologic structures throughout puberty, as puberty allows for the development of Müllerian structures and can reveal anomalies that can impact menstrual and reproductive function. Gynecologic management in puberty may require anatomic and anticipatory counseling only: however, for many patients gynecologic management may require suppression of menses or operative intervention, all of which will be discussed in detail in this article.

Long-term Obstetric and Gynecologic Care for Patients with Anorectal Malformations.
Journal of pediatric surgery
2026: 163294
Abstract
The Pediatric Colorectal and Pelvic Learning Consortium (PCPLC) is a group of medical providers dedicated to equitably improving the health and quality of life of individuals affected by congenital colorectal disorders and associated urologic, gynecologic (GYN), and other anomalies. Using research and medical expertise, the Gynecology Committee within the PCPLC has developed five consensus statements on gynecologic care of patients born with anorectal malformations: Gynecologic Care in Patients with Anorectal Malformations: A Primer and Call to Action, Methods of Gynecologic Evaluation for Patients with Anorectal Malformations, Prepubertal Gynecologic Evaluation and Management of Patients with Anorectal Malformations, Post-pubertal Gynecology Evaluation and Management of Patients with Anorectal Malformations, and Long-term Obstetric and Gynecologic Care for Patients with Anorectal Malformations. In this article, we discuss considerations for long-term gynecologic care including preventive screening, fertility and sexual function as patients transition care from the pediatric to the adult setting. Recommendations for GYN care are based on patient-specific anatomy as well as desires for fertility potential and sexual function. Patient autonomy and shared patient-provider decision-making are the cornerstone of long-term GYN care in this population.

In this article, we will provide an introduction and overview of the subsequent consensus statements while also reviewing the strengths and weaknesses of current research and recommendations for gynecologic care in this population. This article serves as an overview, but also as a call to action for ongoing collaborative clinical care and high-quality research to strengthen care and consensus recommendations going forward.

Methods of gynecologic evaluation for patients with anorectal malformations.
Journal of pediatric surgery
2026: 163291
In this article, we review the clinical, radiologic, and surgical methods used in the evaluation of the gynecologic system. The goal of this manuscript is to equip providers with technical tips and tricks for evaluating the gynecologic system. We will cover basic and advanced methods of gynecologic evaluation as well as options for evaluation in high versus low resource settings to increase accessibility of gynecologic evaluation for all patients.

Pre-pubertal Gynecologic Evaluation and Management of Patients with Anorectal Malformations.
Journal of pediatric surgery
2026: 163292
In this article, we review the timing and recommended methods of gynecologic screening and management in the prepubertal period. During this period, evaluation of gynecologic systems aids the surgical decision-making, and provides anticipatory long-term gynecologic planning. Although stooling and voiding function are high priorities for patients with an ARM in the prepubertal period, evaluation and management of the gynecologic system and gynecology-targeted counseling remain important aspects of comprehensive ARM care.

Abdominopelvic Pain and Overlapping Pain Conditions in Patients with Mayer-Rokitansky-Küster-Hauser Syndrome.
Journal of pediatric and adolescent gynecology
2026
Abstract
Our objectives were to estimate the prevalence of abdominopelvic pain (AP) in patients with Mayer-Rokitansky-Küster-Hauser Syndrome (MRKH), compare the prevalence of AP in patients with anatomic variations and extragenital anomalies in MRKH, and estimate the prevalence of overlapping pain conditions (OPC) in patients with MRKH.Retrospective cohort study of MRKH patients seen at an academic center (2014 - 2024), identified using ICD-10 codes for uterine agenesis (UA) or vaginal agenesis (VA). Inclusion criteria included a confirmed diagnosis of MRKH. Patients with 46,XY karyotypes were excluded. Descriptive statistics were performed. Chi-square analyses were used for categorical variables and two-tailed student's t tests were used for continuous variables to compare features of MRKH patients, including presence/absence of uterine remnants (UR). Multivariate logistic regression was performed to further evaluate associations with AP.117 patients were identified and subcategorized by UR status (UR n=63, no UR n=54), and MRKH type (Type 1 n=65, Type 2 n=52). Overall, 36.8% of patients (n=43) reported persistent AP and 20.5% of patients (n=24) cyclic AP. UR were associated with persistent (p = 0.008) and cyclic AP (p = 0.001). There was no association between MRKH type and persistent (p = 0.732) or cyclic AP (p = 0.282). 23.9% of (n=28) patients were treated for ≥1 OPC, with migraine headache (n=21) and IBS (n=8) being most common.Given the burden of persistent and cyclic AP among MRKH patients, careful and timely evaluation for pain is warranted. Management should focus on clinical presentation over imaging findings.
