Chad Anderson

BioChad Anderson is a Physician Assistant at Stanford ValleyCare and a Clinical Assistant Professor with the Stanford School of Medicine MSPA program. He is the Assistant Director of PA education at ValleyCare. He is dual credentialed as a Family Nurse Practitioner and a Physician Assistant. He completed his FNP/PA training at the Stanford School of Medicine and his graduate studies at A.T. Still University – Arizona School of Health Sciences. He is passionate about improving our patients hospital experience as well as educating our future providers.

Maja Artandi, MD
Clinical Professor, Medicine - Primary Care and Population Health
BioDr. Artandi is a Professor of Medicine in the Division of Primary Care and Population Health at Stanford. She is a leader in Primary Care, spearheading novel methods of health care delivery and education. She offers an impressive clinical background and has received several educational and leadership awards.
Her expertise lies in the development and implementation of a medical curriculum focused on the patient-physician interaction, emphasizing communication skills, physical examination skills and medical decision making to support best clinical practices.
Dr. Artandi is a dedicated Primary Care physician and educator and has served as a mentor for many students, residents and colleagues. She is currently the mentorship lead for the Division of Primary Care and Population Health.
She is in the process of getting an executive coaching certification and is faculty for the Advancing Communication Excellence at Stanford Program with the goal of helping her colleagues improve their communication skills.
From 2013-2021 she was the Co-Director of Primary Care education for the Stanford Internal Medicine residency program and co-founded and co-directed the Primary Care program (ACE) within the Stanford Internal Medicine Residency program.
Dr. Artandi is currently the Co-President of the Society of Bedside Medicine, an international society dedicated to studying and improving the patient/physician interaction.
She is a fellow of the Royal College of Physicians Edinburgh and of the American College of Physicians and currently serves as the Wellness chair for the Northern California ACP chapter. -

Mehrdad Ayati
Adjunct Clinical Associate Professor, Medicine - Primary Care and Population Health
BioDr. Ayati completed his residency at UC Davis and his fellowship at Stanford University. During his residency at UC Davis, Dr. Ayati received the Award of Excellence in Clinical Teaching. Dr. Ayati worked at hospitals such as Lodi Memorial Hospital in Lodi, California, and as an Emergency Medicine Attending at Veteran Affairs in Palo Alto, California. Dr. Ayati worked as a Stanford Medical Director at Los Altos and Palo Alto Subacute and Rehabilitation centers and he is currently serving as a medical advisor for many Skilled Nursing facilities and also memory care units and assisted living facilities in Northen California. Dr. Ayati has a broad spectrum of practice and knowledge of general medicine and primary care in various settings, from office to Emergency room and acute and Sub-acute care. Dr. Ayati’s main areas of research and clinical focus are in the physiology of aging and on finding practical and yet innovative ways of addressing the wellbeing and needs of the population in any age category. Dr. Ayati is an advocate of his patient’s physical and mental health at any age in addition to disease management and prevention. Dr. Ayati is currently a member of the Ethnogeriatric and Quality and Policy Performance Committees of the Americal Geraitric Society. He also serves as a Community Health Advisor for Alzheimer Association, Northen California and Nevada Chapter. Dr. Ayati is the author of “Paths to Healthy Aging”. Dr. Ayati is also a guest educational speaker on several radio stations such as National Public Radio (NPR) and San Francisco’s KQED and international and national conferences. He also testified in the Senate of the US, Special Committee of Aging in 2018 to address the challenges of aging populations in the US.
Dr. Ayati strives to provide reliable information, effective strategies, and simple guidelines for patients of all ages to avoid or manage chronic diseases and to have a significantly better quality of life.
Dr. Ayati’s main focus and passion are in:
Raising awareness about Over Medication and Drug Cascade issues in the elderly population as well as highlighting prevention strategies
Helping patients better understand and voice their end of life care choices and medical intervention wishes
Bringing into focus the numerous social, economical, political and health challenges and hardship the elderly face in our society as well as finding solutions to address their needs

Gordon Bloom
Lecturer, Medicine - Primary Care and Population Health
BioGordon founded the Social Entrepreneurship Collaboratory (SE Labs) at Stanford, Harvard and Princeton. He teaches about the design, development and leadership of innovative social impact ventures in global health and environmental sustainability.
At Stanford, Gordon is director of the Social Entrepreneurship and Innovation Lab (SE Lab)- Human & Planetary Health and is a faculty fellow of the Center for Innovation in Global Health. He is a Lecturer in the School of Medicine, Division of Primary Care and Population Health/Dept. of Medicine, and an advisor in the Distinguished Careers Institute (DCI), and the Knight-Hennessy Scholars Program, and cofounder of the Stanford Sustainable Societies Lab.
At Harvard, Gordon taught jointly on the faculties of the Harvard T. H. Chan School of Public Health (Health Policy & Management) and the Harvard Kennedy School (Management, Leadership & Decision Sciences) and served as an Expert-in-Residence (EiR) at the Harvard Innovation Lab (i-Lab), and affiliated faculty at the Center for Primary Care, Harvard Medical School (HMS). He was faculty director of the Social Entrepreneurship and Innovation Lab (SE Lab) for US & Global Health, an incubator course taught in a new interdisciplinary, collaborative model based at the i-Lab. He has also served as an Entrepreneur-in-Residence (2013-2014) at Harvard Business School in the Rock Center for Entrepreneurship, on the Faculty of Arts & Sciences in the Sociology Department, at the Harvard Kennedy School, on the Leadership & Management faculty, and as a principal of the Hauser Center for Non-Profit Organizations (2004-2007). Gordon served as one of the founding faculty of the $10 million Reynolds Fellows Program in Social Entrepreneurship, a Center for Public Leadership and Harvard President’s interdisciplinary fellowship initiative that paid full tuition and stipend for graduate students from the Harvard Kennedy School, School of Public Health and Graduate School of Education.
At Princeton, Gordon served as Dean’s Visiting Professor in Entrepreneurship in 2009-2010. Working together with the School of Engineering & Applied Science, the [Woodrow Wilson] School of Public & International Affairs, and the Faculty of Arts & Sciences, he launched a new set of programs and prizes in social innovation and entrepreneurship in collaboration with students, faculty and alumni.
At Stanford in 2001-2002, Gordon created the SE Lab, a Silicon Valley and technology–influenced, interdisciplinary incubator for social impact ventures and global problem solving. Gordon taught on the Public Policy Program and Urban Studies Program faculties (School of Humanities & Sciences) and served as a faculty affiliate at the Center for Social Innovation at Stanford Graduate School of Business, and a Program Officer at Stanford’s Freeman Spogli Institute for International Studies.
Many of the talented students and fellows in Gordon’s SE Labs have won the top awards of prestigious idea and business plan competitions, including those at Stanford, Harvard, Princeton and MIT.
Gordon is a co-author in the edited volume Frontiers in Social Innovation (N. Malhotra, ed., Harvard Business Review Press, 2022) and Social Entrepreneurship: New Models of Sustainable Social Change (A. Nicholls, ed., Oxford University Press, 2006/2008) and served as a founding member of the Oxford/Ashoka led University Network for Social Entrepreneurship. His interest in entrepreneurship is informed by work in both the private and nonprofit sectors in the U.S. (New York, Cambridge, Palo Alto), Europe (London, Paris) and Asia (Hong Kong), as CEO of a medical technology company (EDAP Technomed, USA) and in international strategy consulting (Bain & Co. Ltd.).
Gordon is married to Sara Singer- they on occasion teach together at Stanford, have a daughter Audrey and son Jason, and live in the Frenchman's Hill residential section of campus. -
Bryan Bohman
Clinical Professor, Medicine - Primary Care and Population Health
BioBryan Bohman is Associate Chief Medical Officer for Workforce Health and Wellness. Additional roles include Clinical Professor of Medicine and of Anesthesiology, Co-Director of the Clinical Effectiveness Leadership Training (CELT) program and Senior Advisor to the WellMD Center.
Bryan trained at Stanford in internal medicine and anesthesiology. After two decades of clinical practice in community-based anesthesiology, he served as SHC's first elected Chief of Staff from 2008-2011.
As Chief of Staff, Dr. Bohman established Stanford’s wellness committee and subsequently shepherded the founding of its WellMD Center in 2015, serving as the Center’s interim Director until 2017. The Center’s aim is to advance faculty, trainee and care team wellbeing across Stanford Medicine while also serving as an international leader of scholarship in occupational wellbeing. Bryan also led the establishment in 2014 of the CELT program, which continues to serve as a driver of clinical quality improvement across Stanford Medicine.
Dr. Bohman’s primary areas of interest include occupational wellbeing, process improvement, and the reciprocal influences between quality improvement, healthcare system performance, and the occupational wellbeing of healthcare personnel. -
Heather E Boynton
Adjunct Clinical Assistant Professor, Medicine - Primary Care and Population Health
BioHeather E Boynton is an emergency physician at Pioneers Memorial Hospital in Brawley, California and an Adjunct Clinical Assistant Professor at the Stanford University School of Medicine in the Department of Primary Care and Population Health.
Dr. Boynton trained in emergency medicine at UC San Diego, where she served as chief resident. She attended medical school at Georgetown University and also has a master’s degree in International Security Studies from the School of Foreign Service. She completed her undergraduate studies at Princeton University.
As an emergency physician practicing in a rural, cross-border community she hopes to challenge and engage rotating students to provide patient-centered care in a resource-limited setting. -
Michelle Elizabeth Yael Braunschweig, MD, PhD, MPH

BioDr. Braunschweig is a board-certified family medicine doctor. She provides care for the entire family and welcomes patients of all ages.
Her special interests include women’s health, children’s health, and mental health. For each patient, she develops a personalized plan of care. Her goal is to help every individual achieve the best possible health and quality of life.
Another special interest of Dr. Braunschweig is medical education. She is a Clinical Assistant Professor in the Department of Medicine, Division of Primary Care and Population Health, at Stanford University School of Medicine.
The training of health care professionals in low-resource settings is one of her key research interests. Another is reproductive justice.
Dr. Braunschweig grew up in San Jose. Prior to medical school, Dr. Braunschweig studied music and earned a PhD in musicology from UC Berkeley. Her interest in women’s health led her to volunteer as a birth doula at San Francisco General Hospital.
There, she became passionate about maternal and child health, and was inspired to go to medical school to become a doctor.
Outside of her clinical practice and research, she enjoys spending time with her family, going for walks, and trying out new recipes. -
Keri Brenner
Clinical Associate Professor, Medicine - Primary Care and Population Health
BioKeri Brenner, MD, MPA is Clinical Associate Professor of Medicine at Stanford University. As a palliative care physician and psychiatrist, her clinical work includes inpatient palliative care consultations at Stanford. She was inspired to pursue palliative care after serving at Mother Teresa’s Home for the Dying in Kolkata, India on multiple occasions. Dr. Brenner’s scholarly interests and research focus on the psychological elements of palliative care, specifically psychodynamic and existential issues in patients with serious illness. Dr. Brenner completed her medical degree at Yale School of Medicine, where she received honors for her thesis on the phenomenology of suffering with terminal illness. She also has a Master in Public Administration from Harvard’s Kennedy School. Dr. Brenner completed adult psychiatry residency at Massachusetts General Hospital and palliative care fellowship at Harvard. She served on the University of Notre Dame Board of Trustees (2005-2008), and was awarded funding through Harvard’s Radcliffe Institute for Advanced Study (2019). In her personal life, Dr. Brenner enjoys the beautiful outdoors of Northern California with her husband and four young children.

Spencer Chang

BioDr. Spencer Chang is a board-certified Internal Medicine physician who practices at the Stanford Internal Medicine Clinic in Palo Alto. Dr. Chang earned his MD at Duke University School of Medicine where he was part of the Primary Care Leadership Track. He completed his Internal Medicine Residency training at UCLA in the Primary Care Program. Dr. Chang has interests in medical education, improving access to primary care, community health, and Asian Pacific Islander health disparities. He believes in a holistic approach to patient care, addressing the physical, mental, and emotional well-being of his patients and partnering with them to achieve their health goals.

Her interests include primary care, women’s and men’s health, geriatric medicine, mental health, travel medicine, and preventive care.
Dr. Chen values the importance of mutual trust in a patient-doctor healthcare partnership. Elana Trubowitch Cohn

BioDr. Elana Cohn (She/Her) is a board-certified Family Physician, with a love for caring for the whole person and the entire family.
She completed her bachelor's degree in Neuroscience and Behavior at Barnard College, after which she worked as a Health Outreach coordinator on a mobile medical van providing care to the homeless population in NY, which sparked her love for primary care. She received her medical degree at Tel aviv University, and completed her residency at Mount Sinai and the Institute for Family Health.
Her practice spans care for all ages, with an emphasis on care for marginalized populations, reproductive health, office based-procedures, and teaching. She is passionate about integrating mental health care and reproductive justice into general practice, and making care for her patients as comprehensive as possible.
Outside of the office, she loves to travel and explore new places, dance her heart out, and spend time with her family. -

BioDr. Cours is a clinician educator in geriatrics and the clinical director for Aging Adult Services. BioDr. Nancy Cuan is an internal medicine primary care physician at Stanford Coordinated Care (SCC). Korina De Bruyne

Current Research and Scholarly InterestsThe EMPOWER study (PI: Dr Beth Darnall) is looking at how to best support patients with chronic pain on long-term opioid therapy through a slow taper (maximal duration of 1 year). Patients are randomized to taper only versus taper plus community-based pain self-management group sessions versus taper plus psychologist-led cognitive behavioral therapy for pain group sessions. Along the way alternative measures to control pain are also explored. Enrollment is open until 10/2021.

Current Research and Scholarly InterestsMy interest for many years has focused on neuro-oncology (brain tumors) and utilizing both surgery and stereotactically focused radiation to treat solid tumors of the nervous system primarily utilizing the CyberKnife.
In addition, I am an expert in complex and minimally invasive spine surgery.

BioDr. Michelle Engle grew up in Virginia, though she has also lived in China and Canada. She moved to California for medical training and quickly grew attached to the Bay Area. She is board-certified in family medicine and palliative medicine, providing holistic care to patients of all ages.

Aubrey L. Florom-Smith, PhD, RN, AFAsMA

BioAubrey Florom-Smith, PhD, RN, AFAsMA, is a Clinical Assistant Professor in the Department of Medicine, Division of Primary Care and Population Health, at the Stanford University School of Medicine. She is a Nurse Scientist in the Office of Research, Patient Care Services at Stanford Health Care, where she supports Acuity Adaptable Unit nursing and interprofessional research, technology research, and nursing research at Stanford Health Care-TriValley. Dr. Florom-Smith has over 10 years of nursing, clinical, and applied research experience, across a wide range of areas of inquiry, and in healthcare, corporate, and laboratory settings. She obtained her Bachelor of Science in Nursing and her PhD in Nursing Science from the University of Miami, where she was the first National Institutes of Health/National Institute of Nursing Research Predoctoral Fellow at the School of Nursing and Health Studies. Dr. Florom-Smith’s research interests include exploration of novel healthcare delivery models as complex systems, understanding, sustaining and enhancing human health and performance during spaceflight, and adapting impactful countermeasures and interventions for use at the terrestrial bedside and in space. Inducted into Sigma Theta Tau in 2008, Dr. Florom-Smith has received several honors and awards, including the 2020 University of Miami Alumnus of Distinction Award, the Aerospace Nursing and Allied Health Professionals Society Louise Marshall Nursing Scholarship, the Jonas Foundation Nurse Leader Scholar, the Sigma Theta Rising Star of Scholarship and Research Award, and the March of Dimes Rising Star Award. Dr. Florom-Smith is an Associate Fellow of the Aerospace Medical Association.

Andrea Fox

BioAndrea is a certified Physician Assistant who joined Stanford Health Care in 2019 to develop a comprehensive Fracture Liaison Service and bone health clinic championed by Dr. Michael Gardner, Trauma Orthopaedist. Andrea earned a Masters degree in Medical Science, Physician Assistant Studies and a Masters in Health Administration, both from the University of Missouri. She holds a current certification with the International Society of Clinical Densitometry and has completed her certificate trained as a fracture liaison clinician through the National Osteoporosis Foundation. She has a keen interest in educating primary care providers and the general public on the importance of early monitoring, risk factors and treatment for bone loss and the prevention and healing of fractures.

Clinical Associate Professor, Medicine - Primary Care and Population HealthBioDr. Fukaya practices Vascular Medicine at the Stanford Vascular Clinics and Advanced Wound Care Center. She received her medical education in Tokyo and completed her medical training both in the US and Japan. She joined Stanford in 2015.
Vascular Medicine covers a wide range of vascular disorders including chronic venous insufficiency, varicose veins, deep vein thrombosis, post thrombotic syndrome, peripheral artery disease, carotid artery disease, cardiovascular risk evaluation, fibromuscular dysplasia, rare vascular disease, lymphedema, arterial/venous/diabetic ulcers, and wound care.
Dr. Fukaya has a special interest in venous disease and started the Stanford Vascular and Vein Clinic in 2016.

Pascal Geldsetzer

BioPascal Geldsetzer is an Assistant Professor of Medicine in the Division of Primary Care and Population Health and, by courtesy, in the Department of Epidemiology and Population Health. He is also affiliated with the Department of Biomedical Data Science, Department of Health Policy, King Center for Global Development, and the Stanford Centers for Population Health Sciences, Innovation in Global Health, and Artificial Intelligence in Medicine & Imaging.
His research focuses on identifying and evaluating the most effective interventions for improving health at older ages. In addition to leading several randomized trials, his methodological emphasis lies on the use of quasi-experimental approaches to ascertain causal effects in large observational datasets, particularly in electronic health record data. He has won an NIH New Innovator Award (in 2022), a Chan Zuckerberg Biohub investigatorship (in 2022), and two NIH R01 grants as Principal Investigator (both in 2023). -
Linda N. Geng, MD, PhD

Current Research and Scholarly InterestsMy scholarly interests are focused on defining, studying, and improving patients' diagnostic journeys. What prolongs the journey to the correct diagnosis and how can we shorten it? With this question in mind, we are exploring crowdsourcing, informatics/AI, health data visualization, and advanced laboratory testing as ways to help tackle the toughest cases in medicine-- complex, rare, and mystery conditions.
With the COVID pandemic, the puzzling and complex illness of post-acute COVID-19 syndrome (PACS) or long COVID came to light. Together with a multidisciplinary group of physicians and researchers, we launched a program here at Stanford to advance the care and understanding of PACS. Our goal is to better understand the natural history, clinical symptomatology, immunological response, risk factors, and subgroup stratification for PACS. We are also actively assessing management strategies that may be effective for heterogeneous PACS symptoms. -

Assistant Professor of Medicine (Primary Care and Population Health)
BioResearch Focus: In contrast to bounded teams with static membership, dynamic teaming reflects the common challenge of interdisciplinary healthcare teams with changing rosters. Such dynamic collaboration is critical to addressing multi-faceted problems and individualizing care. At present, off the shelf interventions to improve the way healthcare teams work - often assume static and bounded teams. I intend to leverage design approaches to build a new kind of healthcare “teaming intervention,” which respects the nature of their constantly changing membership and more closely aligns with how healthcare teams actually collaborate.
Expertise: My expertise includes organizational behavior, building interdisciplinary teams, implementation science, mixed methods-research, quality improvement, pain and palliative care research, and global health.
Positions: I am an Instructor in the Division of Primary Care and Population Health at Stanford University School of Medicine and a Core Investigator at the Center for Innovation to Implementation (Ci2i) in the VA Palo Alto Health Care System where I am PI or co-investigator on multiple ongoing studies representing over 25 million dollars of competitive government grant funding. I am also a Director of the VA Quality Improvement Resource Center (QuIRC) for Palliative Care, supporting Geriatrics and Extended Care programs for 170 Veterans Affairs facilities nationally. In QuIRC I lead a portfolio of projects on improving the processes that interdisciplinary teams can leverage to improve pain and symptom management among high-risk patients; specifically I’m aiming to bridge the gap of poor palliative care integration in the perioperative period.
Accomplishments: I have over 50 peer reviewed publications in high quality medical and health services delivery journals such as Medical Care, JAMA Surgery, the Journal of General Internal Medicine, the Journal of Pain and Symptom Management and Pain Medicine. I recently received a 5-year VA Career Development Award on building better teams across disciplines and am an American Academy of Hospice and Palliative Medicine Research Scholar for related work. -
Alan M Glaseroff
Adjunct Professor, Medicine - Primary Care and Population Health
BioDr. Alan Glaseroff served as the Director of Workforce Transformation in Primary Care at Stanford from the fall of 2015 until mid-June of 2016, where he was responsible for training the teams for Primary Care 2.0, a radical redesign of primary care underway in 2016. He will be joining the faculty at Stanford's Clinical Excellence Research Center this summer, working with Dr. Arnie Milstein to help develop new models of care. He formerly served as Co-Director of Stanford Coordinated Care, a service for patients with complex chronic illness from 2011 to the end of 2015. Dr. Glaseroff, a member of the Innovation Brain Trust for the UniteHERE Health, currently serves as faculty for the Institute of Healthcare Improvement’s “Better Care, Lower Cost” collaborative and served as a a Clinical Advisor to the PBGH “Intensive Outpatient Care Program” CMMI Innovation Grant that completes in June 2015. He served on the NCQA Patient-Centered Medical Home Advisory Committee 2009-2010, and the “Let’s Get Healthy California” expert task force in 2012,. Dr. Glaseroff was named the California Family Physician of the Year for 2009.
Dr. Glaseroff’s interests focus on the intersection of the meaning of patient-centered team care, patient activation, and the key role of self-management within the context of chronic conditions.

Current Research and Scholarly InterestsI am interested in issues relating to the representation and measurement of evidence in medical research, and determinants of the truth of medical findings, using a Bayesian framework. I also do work in evidence synthesis, comparative effectiveness research, and the ethics of clinical research.

Lindsey Merrihew Haddock
Clinical Assistant Professor, Medicine - Primary Care and Population Health
BioLindsey Haddock, MD, MAEd, is a geriatrician and clinician-educator with a master's degree in education. Her research in medical education focuses on learning in the clinical workplace and evaluation of workplace-based assessments. She is the co-director of Primary Care and Population Health's Quality Education Scholarship Training program (QuEST), and the associate program director of the fellowship in Geriatrics. She works clinically in Stanford Senior Care Clinic and the inpatient geriatrics service.

Kurt M. Hafer, MD, FACP
Clinical Associate Professor, Medicine - Primary Care and Population Health
BioDr. Kurt Hafer is a board-certified physician and Fellow of the American College of Physicians (FACP) practicing Primary Care Internal Medicine exclusively at Stanford Concierge Medicine.
Dr. Hafer grew up in Chapel Hill, NC and attended Pomona College, where he received his undergraduate degree in Psychology. After completing post-baccalaureate pre-medical coursework at the University of Michigan (UM) in Ann Arbor, he worked as a neuro-endocrine peptide researcher at UM.
In 1999, Dr. Hafer graduated from The University of Chicago Pritzker School of Medicine. He completed a Primary Care Internal Medicine Residency at Santa Clara Valley Medical Center (SCVMC) in San Jose in 2002. Between 2002 to 2012 he was a Teaching Attending Physician at SCVMC as well as an adjunct Stanford physician, training medical students and residents in Internal Medicine.
Dr. Hafer joined Stanford in 2012 as the founding Medical Director of the Stanford Primary Care, Portola Valley Clinic -- Stanford's first new primary care clinic in many years. His five years of leadership at the Portola clinic included incorporating the latest technologies into primary care, adopting active population health panel management, LEAN management practices, embedded specialists and evidence-based, best-care practices as a viable model for the future of Stanford Primary Care.
In January 2017, Dr. Hafer joined Stanford Concierge Medicine as Medical Director. In addition to caring for his patients, his duties include directing the clinic and expanding clinic offerings in mental health, wellness, and piloting Primary Care Genetics and Pharmacogenomics screening programs as a test bed for Stanford Primary Care.
While at Stanford, Dr. Hafer has served as a lecturer for the American College of Physician's Internal Medicine Maintenance of Certification Course held in San Francisco, and has been a Reviewer for the American College of Physicians on multiple projects. He has served on numerous Stanford Healthcare committees and worked with teams on numerous projects, including Stanford's Primary Care 2.0 Redesign, Hypertension Center of Excellence Clinical Integration Team, The Virtual Hypertension Monitoring Project, and Stanford's Primary Care Precision Health program design team. He has directed pilots of TeleHealth phone and video visits, integration of specialty care MDs into our primary care clinics. He led a successful Clinical Effectiveness Leadership Training (CELT) project using clinical pharmacists embedded in primary care clinics to more effectively manage diabetes and high blood pressure between MD visits. He has also served as the Physician Leader for Stanford's Realizing Improvement through Team Empowerment (RITE) Quality Improvement Program.
He currently serves as a Physician Member and Chair (2023) of the Global Executive Services (GES) Network Steering Committee, part of the Vizient University Health System Consortium, a national group of ~200 members of academic medical centers with Executive Health or Concierge Medicine services.
When not caring for patients, Dr. Hafer enjoys spending time outdoors with family and friends. He is married to a Stanford University History Professor, has a daughter who graduated from Stanford and UCLA Medical School (now a resident at UCSF), as well as a son who is studying computer science at Stanford. He is an avid lifelong cyclist (road and MTB, logging over 8k miles annually), hiker, has a passion for tinkering with vintage Datsuns and enjoys wearing vintage watches.
Dr. Hafer believes that a combination of truly knowing his patients as individuals, excellent patient-physician communication, and comprehensive preventive care allows him to provide exceptional care for his patients. -

Josef Hannah
Clinical Assistant Professor, Medicine - Primary Care and Population Health
BioDr. Hannah graduated from Kansas City University of Medicine & Biosciences. He then completed a residency in Internal Medicine at the University of Kansas and a fellowship in Hospice & Palliative Medicine at Stanford University before joining as faculty at Stanford. His clinical practice includes both inpatient palliative care consultation as well as ambulatory care in palliative medicine. His research and educational interests include symptom management and utilizing media to grow palliative care services and education.

Stephanie Harman
Clinical Associate Professor, Medicine - Primary Care and Population Health
BioDr. Stephanie Harman is a palliative care physician and Clinical Associate Professor of Medicine. She graduated from Case Western Reserve University School of Medicine and went on to complete a residency in Internal Medicine at Stanford and a Palliative Care fellowship at the Palo Alto VA/Stanford program. She then joined the faculty at Stanford. She co-founded the Palliative Care Program at Stanford Health Care in 2007 and served as Clinical Chief of the Section of Palliative Care in the Division of Primary Care and Population Health from 2016 - 2022. She is the inaugural Associate Chair for Women in Medicine for the Department of Medicine and the Director of the Stanford Leadership Development Program, a joint program between Stanford Health Care and the School of Medicine for emerging leaders. A 2017 Cambia Health Foundation Sojourns Scholar Leader Awardee, she has a passion for leadership development and has built a Women Leaders in Academic Medicine (WLAM) program in the Division of Primary Care and Population Health, which is now in its 6th year with 28 women leaders. Her other professional interests include clinical ethics and serious illness communication.

Shireen N. Heidari
Clinical Associate Professor, Medicine - Primary Care and Population Health
BioShireen Heidari, MD is a palliative care and family medicine physician. She works as part of the inpatient palliative care consult team providing symptom management and support for patients and families facing any stage of a serious illness. Dr. Heidari is the program director for the Stanford University Hospice and Palliative Fellowship. She previously served as the clinical lead for the Stanford site of the PERIOP-PC Study, collaborating with the surgical department to evaluate the impact of early palliative care support for patients and family members preparing for major upper gastrointestinal cancer surgery.
Dr. Heidari has written about the importance of human connection and stigma around healthcare workers seeking help for their mental health in The New England Journal of Medicine, The Lancet Respiratory Medicine, and The Intima. She hopes that by sharing her own story, she can continue being part of this conversation as we advocate for culture change in medicine and more sustainable practice.
Before moving to northern California, Dr. Heidari attended medical school at Boston University, completed her residency at UC San Diego where she served as chief resident, followed by palliative fellowship at UCLA. Outside of her clinical and mentorship work, she is likely writing creatively or outside with her husband chasing their dogs. -
Heather Henri, MD
Clinical Associate Professor, Medicine - Primary Care and Population Health
BioDr. Heather Henri is an Associate Professor at the Stanford University School of Medicine.
Dr. Henri attended Stanford University, where she received her undergraduate degree in Biological Sciences. At Stanford she was the recipient of the President's Award for Academic Excellence in the Freshman Year, awarded to the top 3% of students. She has been the recipient of an American Heart Association Internship at the National Heart Lung and Blood Institute of the National Institutes of Health (NIH). She also was awarded two Howard Hughe's Institute Research Fellowships at the NIH, one at the National Cancer Institute and the second at the National Eye Institute.
In 2000, Dr. Henri graduated from Harvard Medical School. She then completed a Surgical Internship at Stanford Medical Center, followed by an Internal Medicine Residency at Stanford in 2004. She served as the Clinic Chief at Blake Wilbur of Stanford Medical Group from 2006-2009, as well as on the General Internal Medicine Executive Committee at Stanford between 2006-2009.
Dr. Henri was named as a "Top Reviewer" based on the quality of reviews by the Annals of Internal Medicine in 2006. With Dr. Peter Rudd, she co-authored the chapter "Hypertension: Context and Managment" in the Textbook of Cardiovascular Medicine published in 2007. Between 2006-2007 Dr. Henri was a member of the California Academic Collaborative on Chronic Care.
In 2013, Dr. Henri joined Stanford Concierge Medicine where she practiced for six years. She also served as Director of the American College of Physician's Internal Medicine Maintenance of Certification Course held in San Francisco from 2013-2017. Dr. Henri has been invited by the American College of Physicians to work on multiple projects. She served on the Internal Medicine Essentials Text and Online Questions Editorial Review Board for the American College of Physicians. In 2016, 2017 and 2018 Dr. Henri reviewed and authored educational material for the American College of Physician's Medical Knowledge and Self Assessment Program (MKSAP 17) Updates. Starting in 2018, Dr. Henri was a primary regular reviewer of educational podcasts published by the American College of Physicians. In 2023 Dr. Henri served as a content Editor for the American College of Physicians Board Preparation course.
Dr. Henri feels that comprehensive preventive care, combined with excellent patient-physician communication, is fundamental to her care of patients.
